Spironolactone is a potassium-sparing diuretic and blood pressure medication. It is used to treat high blood pressure, certain types of swelling, heart failure, and primary hyperaldosteronism.

What it is used for

Spironolactone is indicated for:

  • High blood pressure that is not adequately controlled with other medications;

  • Swelling and ascites caused by liver cirrhosis;

  • Swelling associated with nephrotic syndrome when treatment of the underlying condition, fluid and sodium restriction, and other diuretics have not provided an adequate response;

  • NYHA class III or IV heart failure with reduced ejection fraction;

  • Short-term treatment before surgery for primary hyperaldosteronism;

  • Long-term treatment of primary hyperaldosteronism in people who cannot undergo surgery.

6. Low potassium levels

Because spironolactone reduces potassium loss, a healthcare provider may use it when treatment with other diuretics causes low blood potassium levels.

7. Primary hyperaldosteronism

Medical attention should be sought immediately if symptoms of abnormal potassium or other electrolyte levels develop, such as:

  • Muscle weakness;

  • Tingling sensations;

  • Difficulty moving part of the body;

  • Muscle pain;

  • Chest pain;

  • An irregular heartbeat;

  • Increased thirst;

  • Increased urination;

  • Severe fatigue;

  • Confusion.

Emergency medical care is also necessary if signs of a severe allergic or skin reaction occur, such as difficulty breathing, swelling of the face or throat, a widespread rash, blistering, or peeling skin.

Who should not take it

Spironolactone should not be used by anyone who is allergic to spironolactone or any other ingredient in the medication.

It is also contraindicated in people with:

  • High blood potassium levels;

  • Concomitant use of eplerenone.
