Lower stomach pain during the third trimester can have several causes. Some discomfort may come from pelvic girdle pain, fetal movement, or irregular contractions, but lower abdominal pain can also be related to problems that need prompt medical assessment.
Pain or pressure in the lower abdomen may occur with preterm labor, especially before 37 weeks. Regular contractions, a dull backache, pelvic pressure, changes in vaginal discharge, vaginal bleeding, or leaking fluid are important symptoms to watch for. Urinary infections can also cause lower abdominal discomfort, sometimes along with urinary symptoms.
Seek urgent maternity care if the pain is severe, persistent, or getting worse, or if it occurs with bleeding, fluid leakage, fever, regular contractions, reduced fetal movement, headache, vision changes, or upper abdominal pain. These symptoms should not be assumed to be a normal part of late pregnancy.
If the pain is new or concerning, contact your obstetrician or maternity care team for assessment. Avoid taking medication for the pain without checking with your healthcare provider first.
