Transaminitis: Symptoms. Causes, Diagnosis & Treatment

Transaminitis is an increase in the blood levels of liver enzymes called transaminases, mainly alanine aminotransferase (ALT) and aspartate aminotransferase (AST). It is a laboratory finding rather than a disease and may indicate irritation or injury involving the liver or, less commonly, another part of the body.

Many people with transaminitis have no symptoms, especially when the enzyme increase is mild. Common causes include metabolic dysfunction-associated steatotic liver disease, alcohol-related liver disease, viral hepatitis, medication-related liver injury and certain genetic or autoimmune conditions.

Treatment depends on what is causing the abnormal test results. Medical evaluation is important when the enzyme elevation persists, is severe or occurs with other abnormal liver tests or signs of significant liver disease.

Main symptoms

Transaminitis usually does not cause symptoms by itself, and mild elevations are often discovered during routine blood testing.

The most common clinical findings include:

  • Symptoms related to the underlying liver or health condition

  • Abnormal ALT or AST levels found during routine testing

  • Signs of liver disease on a medical examination when liver damage is more advanced

Symptoms cannot show how high the liver enzymes are or identify the cause. Blood tests and a medical evaluation are needed to understand the finding.

Possible causes

Transaminitis has many possible causes, which can include:

1. Metabolic dysfunction-associated steatotic liver disease

Metabolic dysfunction-associated steatotic liver disease, or MASLD, is a leading cause of persistent mild transaminitis. It involves excess fat in the liver and is commonly associated with obesity, insulin resistance, type 2 diabetes and abnormal blood lipid levels.

Some people with MASLD have no symptoms and are diagnosed after routine blood tests show elevated ALT or AST. Further evaluation may be needed to determine whether liver inflammation or fibrosis is present.

2. Alcohol-related liver disease

Regular or excessive alcohol consumption can injure liver cells and raise transaminase levels. A medical history should include the amount and frequency of alcohol intake because this information helps determine whether alcohol may be contributing to the abnormal results.

The pattern of liver enzyme elevation may provide useful information, but it cannot confirm alcohol-related liver disease by itself. Other causes may need to be investigated at the same time.

3. Viral infections

Viral hepatitis can cause transaminitis because the infection produces inflammation in the liver. Testing for hepatitis B and hepatitis C may be included when investigating persistent or unexplained elevations.

Other infections, including HIV and COVID-19, have also been associated with elevated transaminases in some patients. In these situations, the abnormal enzymes may be related to the infection, the severity of illness, medications or more than one factor.

4. Medications and supplements

Prescription medicines, over-the-counter products, herbal remedies and dietary supplements can cause liver injury. A complete list of all products being used is therefore an important part of the medical evaluation.

A medicine should not be stopped without medical advice unless urgent care has been recommended. When drug-induced liver injury is suspected, the clinician assesses the timing of exposure, the pattern of enzyme elevation and whether other causes are more likely.

5. Less common liver conditions

Persistent transaminitis may be caused by autoimmune hepatitis, hereditary hemochromatosis or Wilson disease. These conditions are less common but may require specific blood tests, genetic tests or specialist evaluation.

The investigation is guided by age, family history, symptoms, the degree of enzyme elevation and other laboratory findings.

6. Conditions outside the liver

AST is also present in tissues outside the liver, including muscle. Muscle disorders may therefore increase AST and sometimes ALT without primary liver disease.

Thyroid disorders and celiac disease are additional possible causes of elevated transaminases. These conditions may be considered when the initial liver evaluation does not identify an explanation.

Confirming a diagnosis

Transaminitis is identified through blood tests showing ALT or AST above the laboratory reference range. Because reference limits can vary, clinicians consider the actual result, the degree of elevation, previous test results and whether other liver measurements, such as alkaline phosphatase or bilirubin, are abnormal.

The evaluation usually includes a review of alcohol intake, medications, supplements, metabolic risk factors, infections, family history and possible exposure to liver toxins. The clinician may repeat the liver tests and request tests for viral hepatitis, iron overload, autoimmune liver disease or other suspected conditions.

An abdominal ultrasound may be used to look for fat accumulation or structural changes in the liver. Noninvasive fibrosis scores can help estimate the risk of significant liver scarring, particularly in people with MASLD. Specialist assessment or liver biopsy may be considered when the elevation persists and noninvasive testing does not establish the cause.

Treatment options

Treatment is directed at the underlying cause of transaminitis rather than the laboratory result alone.

1. Lifestyle changes

Lifestyle measures are commonly recommended when MASLD is suspected. These may include gradual weight loss, regular physical activity, a balanced diet and management of diabetes, blood pressure and abnormal cholesterol or triglyceride levels.

Reducing or avoiding alcohol may be advised when alcohol is contributing to the liver enzyme elevation. Liver tests are usually monitored to determine whether they improve after these changes.

2. Reviewing medications and supplements

When a medicine, herbal remedy or supplement may be causing liver injury, a clinician may recommend stopping or replacing the suspected product. The decision depends on the severity of the abnormal results, the importance of the treatment and the availability of safer alternatives.

Follow-up testing is used to confirm whether liver enzymes improve. Severe or worsening injury may require urgent specialist assessment.

3. Treating the underlying condition

Viral hepatitis, autoimmune hepatitis, iron overload and other specific diseases require treatment appropriate to the diagnosis. Managing the underlying condition can reduce liver inflammation and help transaminase levels return toward normal.

Elevated enzymes related to thyroid, celiac or muscle disorders may improve when the condition outside the liver is treated. Treatment should therefore not be started until the likely cause has been identified.

4. Monitoring

Mild transaminitis may be monitored when the initial evaluation does not show severe liver disease. Repeat blood testing helps determine whether the abnormality is temporary, improving or persistent.

Further imaging, fibrosis testing or referral to a liver specialist may be needed if enzyme levels remain elevated. Monitoring plans vary according to the suspected cause and the person’s overall risk.
