A pinched nerve in the shoulder occurs when a nerve near the shoulder, or a nerve root in the neck, becomes compressed or irritated. The term may refer to local shoulder nerve entrapment, such as compression of the suprascapular or axillary nerve, or cervical radiculopathy that causes pain to spread from the neck into the shoulder.
Common symptoms include shoulder pain, tingling, numbness, muscle weakness and difficulty moving the arm. The exact pattern depends on which nerve is affected, and symptoms may worsen with overhead movements or certain neck positions.
Treatment usually begins with rest, activity changes, medication and physical therapy. Medical evaluation is important when pain persists, weakness develops or symptoms interfere with normal arm movement, as prolonged nerve compression can sometimes lead to lasting muscle weakness.
Main symptoms
The symptoms of a pinched nerve in the shoulder vary according to the location and severity of the nerve compression. Common symptoms include:

Local shoulder nerve compression often causes pain around the back or outer part of the shoulder, while cervical radiculopathy may also cause neck pain and symptoms that extend farther down the arm.
Causes
A pinched nerve in the shoulder is not contagious and cannot be transmitted from one person to another. It develops when a nerve is compressed, stretched or irritated by nearby tissues.
Cervical radiculopathy
Cervical radiculopathy occurs when a nerve root in the neck becomes irritated or compressed. Age-related changes in the cervical spine, bone spurs and problems involving a cervical disc can reduce the space around a nerve root.
Pain may begin in the neck and spread into the shoulder, arm or hand. Tingling, numbness and weakness may also occur in areas supplied by the affected nerve.
Suprascapular nerve entrapment
The suprascapular nerve helps control muscles that move and stabilize the shoulder. It can become compressed at narrow areas near the shoulder blade, especially in people who repeatedly perform overhead movements.
Possible causes include repetitive traction, rotator cuff injuries, shoulder instability and cysts that form near the shoulder joint. Athletes involved in sports such as volleyball, baseball or swimming may have a higher risk because of repeated overhead arm motion.
Axillary nerve compression
The axillary nerve supplies sensation over part of the outer shoulder and helps control the deltoid muscle. Compression may occur in the quadrilateral space, an area at the back of the shoulder through which the nerve travels.
Repetitive overhead activity, injury, swelling or nearby structural abnormalities may place pressure on the nerve. Symptoms can include outer shoulder pain, numbness and weakness when raising the arm.
Injuries and structural abnormalities
Shoulder dislocation, fractures, surgery or direct trauma can injure or compress nerves around the shoulder. Scar tissue or swelling after an injury or procedure may also place pressure on a nerve.
Paralabral cysts, tumors and other masses can occasionally compress a shoulder nerve. Large or retracted rotator cuff tears may also stretch or affect the suprascapular nerve.
Diagnosis
Diagnosis usually begins with a review of the symptoms, recent injuries, work activities, sports participation and movements that make the pain worse. The doctor may examine the neck and shoulder, compare muscle strength on both sides and check sensation, reflexes and shoulder movement.
Physical examination tests can help determine whether symptoms are coming from the neck or from a nerve near the shoulder. Doctors look for characteristic patterns of pain, weakness, numbness, muscle wasting and symptoms triggered by specific neck or arm positions.
MRI may be used to identify cervical disc problems, rotator cuff injuries, cysts, tumors or muscle changes caused by nerve damage. Ultrasound can show nerve enlargement or compression, while electromyography and nerve conduction studies help evaluate how well the nerve and affected muscles are working.
Treatment options
Treatment depends on the cause, severity and duration of symptoms. Conservative treatment is usually tried first unless there is a major injury, severe weakness or a clearly defined mass compressing the nerve.
Rest and activity changes
Temporary rest from painful movements can reduce irritation around the nerve. Repetitive overhead activity, heavy lifting and positions that increase symptoms may need to be limited while the nerve heals.
Complete inactivity is not usually necessary. Gradual return to normal movement may be recommended as pain and strength improve.
Medication
Nonsteroidal anti-inflammatory drugs may be used to help control pain and inflammation. The evidence for these medications in cervical radicular pain is limited, but they are commonly included as part of early conservative care.
Some patients may receive an injection near the affected area to reduce inflammation and pain. Injections do not remove every cause of compression and should be considered according to the diagnosis and overall clinical findings.
Physical therapy
Physical therapy may include exercises to improve posture, shoulder blade control, flexibility and strength. Treatment can also focus on reducing strain on the neck and shoulder during work, sports and daily activities.
For cervical radiculopathy, therapy may include carefully selected neck exercises and manual techniques. For local shoulder nerve entrapment, rehabilitation may focus on shoulder stability, rotator cuff strength and correcting movement patterns that place tension on the nerve.
Nerve blocks and other procedures
A local anesthetic nerve block may help confirm which nerve is causing the pain and may also provide temporary symptom relief. Some specialist treatments use image guidance to place medication close to an irritated nerve.
Procedures such as epidural corticosteroid injections or pulsed radiofrequency may be considered for selected cases of cervical radicular pain. Their benefits and risks should be evaluated individually.
Surgery
Surgery may be recommended when symptoms do not improve with conservative treatment, weakness is progressing or imaging shows a clear structure compressing the nerve. Examples include a cyst, tight ligament, tumor or other lesion that is pressing directly on the nerve.
The procedure may release the nerve, remove a cyst or treat an associated shoulder injury. Cervical spine surgery may be considered when persistent nerve root compression causes severe pain, neurological loss or functional limitation.
When to see a doctor
Medical evaluation is recommended when shoulder pain is persistent, repeatedly returns or is accompanied by numbness, tingling or weakness. Symptoms that interfere with lifting the arm, gripping objects, sleeping or performing normal activities should also be assessed.
Prompt evaluation is especially important after a shoulder dislocation, fracture, major fall or other traumatic injury. Progressive weakness, visible muscle wasting or loss of sensation may indicate significant nerve dysfunction and should not be ignored.
Prevention measures
Not every case of nerve compression can be prevented, especially when it is caused by an injury, cyst or age-related spinal changes. However, reducing repeated strain on the neck and shoulder may lower the risk of some forms of nerve irritation.
