Why does my body ache and I feel tired (with no fever)?

“I’ve been feeling really worn out lately, and my muscles ache even though I don’t have a fever or feel like I’m coming down with anything. Why does my body ache and I feel tired without a fever?”

Body aches with tiredness but no fever can happen for several reasons. It may follow a viral illness after the fever has already gone away, or it may be related to ongoing fatigue and pain conditions such as chronic fatigue syndrome, fibromyalgia, or Long COVID.

These conditions can cause muscle or joint aches, low energy, poor sleep, and feeling worse after normal activity, even when routine signs like fever are absent. Some people also notice brain fog, dizziness, or a flu-like feeling without actually having a fever.

Other health issues can cause similar symptoms, including thyroid problems, vitamin B12 deficiency, sleep apnea, inflammatory conditions, neurologic problems, or medication reactions. Because many causes can look alike, it is important not to assume it is just stress, lack of sleep, or a minor illness if symptoms are persistent.

Seek medical evaluation if the aches and fatigue last more than a few days, keep coming back, worsen after activity, or interfere with daily life. A primary care doctor can review symptoms, medications, sleep, recent infections, and decide whether testing or referral to a rheumatologist, neurologist, or sleep specialist is needed. Avoid self-medicating without guidance.
