Sleeping naked can be healthy if it feels comfortable and helps prevent overheating during the night. However, there is no strong evidence that it is healthier than wearing pajamas or that it directly improves sleep for everyone.

Body temperature and sleep are closely connected. A bedroom that feels too warm can interfere with sleep, so removing sleepwear may help some people stay comfortable. Others may sleep better in loose, breathable clothing, especially because becoming too cold can also disturb sleep.

Sleeping without underwear may reduce irritation for people with ongoing vulvar discomfort, but it has not been proven to prevent yeast infections in the general population. Claims that sleeping naked improves male fertility are also uncertain. Research on underwear choice has found mixed results and has not shown a clear improvement in the chances of pregnancy.

Choose clean bedding and keep the bedroom at a comfortable temperature. Loose cotton or moisture-managing sleepwear is a reasonable alternative when sleeping naked feels uncomfortable. Persistent genital itching, pain, discharge, or sleep problems should be evaluated by a primary care doctor, gynecologist, urologist, or sleep specialist rather than treated without medical guidance.
