Blood in sperm, known as hematospermia, is rarely a sign of cancer. It most often happens due to inflammation, infection, or irritation in the prostate, seminal vesicles, or urethra. It can also appear after certain medical procedures, like a prostate biopsy, or from minor trauma during sex or exercise.

Cancer is found in only a small number of cases, mainly in men over 40 or those who notice repeated episodes. When blood in sperm is linked to prostate cancer, it’s usually accompanied by other signs, such as difficulty urinating, pelvic pain, or an elevated PSA level. If the blood appears only once and disappears, there’s usually no need for concern. But if it continues for more than a month, comes back often, or happens with symptoms like fever, pain, or urinary problems, it’s important to see a doctor for evaluation.
