A heterogeneous myometrium means the muscular layer of the uterus appears uneven or irregular on an ultrasound. This finding often indicates conditions such as fibroids or adenomyosis.

The myometrium is the thick, muscular middle layer of the uterus, while the endometrium (the inner layer) and the perimetrium (the outer layer) are much thinner. Adenomyosis happens when endometrial tissue grows into the myometrium, which can cause the uterus to enlarge and look irregular.

In these situations, the ultrasound may also show small cysts and thickening or irregularities in the junction between the uterine layers, which are strong signs of adenomyosis.
